ABSTRACT

ObjectiveTo observe the clinical efficacy of Danzhi Jiangtang capsules with the functions of replenishing Qi, nourishing Yin, and dredging collaterals on patients with type 2 diabetes mellitus (T2DM) combined with lower-extremity macroangiopathy and serum levels of homocysteine (Hcy) and cystatin C (Cys C). MethodA total of 80 eligible patients who were treated in the department of endocrinology of the First Affiliated Hospital of Anhui University of Chinese Medicine from May 2017 to December 2019 were randomized into the treatment group (40 cases) and control group (40 cases). Both groups received the basic therapies for diabetes and Danzhi Jiangtang capsules (oral) was added to the treatment group. The levels of glycosylated hemoglobin A1c (HbA1c), fasting plasma glucose (FPG), 2-hour postprandial glucose (2 hPG), fasting C-peptide (C-P), and 2-hour postprandial C-peptide (2 hC-P), triglycerides (TG), total cholesterol (TC), high density lipoprotein cholesterol (HDL-C), and low density lipoprotein cholesterol (LDL-C) and serum levels of Hcy and Cys C were measured and the traditional Chinese medicine(TCM) syndrome scores were calculated before and after treatment in the two groups. ResultAfter 12 weeks of treatment, levels of HbA1c, FPG, and 2 hPG were lower than those before treatment in the two groups (P<0.05). Levels of C-P (P<0.05) and 2 hC-P were higher than those before treatment in the two groups. After treatment, levels of HbA1c, FPG, and 2 hPG in the treatment group were lower than those in the control group (P<0.05), while levels of C-P and 2 hC-P showed no significant difference between two groups. After treatment, the levels of TG, TC, and LDL-C were lower than those before treatment (P<0.05) and HDL-C level was higher than that before treatment (P<0.05) in both groups. After treatment, levels of TG and LDL-C in the treatment group were lower than those in the control group (P<0.05), and levels of TC and HDL-C demonstrated no significant difference between two groups. After the treatment, the TCM syndrome score was lower than that before the treatment in both groups (P<0.05) and lower in the treatment group than in the control group (P<0.05). The overall effective rate of the treatment group was higher than that of the control group (χ2=7.585, P<0.05). The levels of Cys C and Hcy were lower than those before treatment in the two groups (P<0.05) and lower in the treatment group than in the control group (P<0.05). Doppler echocardiography of the lower limbs showed no obvious improvement in the control group after treatment. However, for the treatment group, slight decrease in intima-media thickness of the lower limb arteries and a slight reduction in the plaque area were observed, but the difference was not statistically significant. ConclusionDanzhi Jiangtang capsules has definite therapeutic effect on T2DM combined with lower-extremity macroangiopathy, which can improve glucolipid metabolism and reduce serum levels of Hcy and Cys C. This study can serve a reference for the prevention and treatment of T2DM combined with macroangiopathy.

ABSTRACT

Depression is a mental or affective disorder, whose occurrence is closely related to the physical, psychological, and social environmental factors. Clinically, patients with depression often present with significant and long-lasting blue mood, impaired thinking and cognitive functions, reduced mobility, and even self-harm and suicide attempts. The pathogenesis of depression is complex and diverse, and there mainly exist the neurotransmitter hypothesis, immunodeficiency hypothesis, hypothalamus-pituitary-adrenal (HPA) axis activation hypothesis, brain-derived neurotrophic factor (BDNF) hypothesis, and intestinal flora hypothesis. In western medicine, it is mostly treated with antidepressant drugs, but the resulting side effects and repeated attack cannot be ignored. Traditional Chinese medicine(TCM) has obvious advantages in dealing with depression. The advocates of treating "Qi", "phlegm", "stasis", and "five zang organs" have all been proved effective. Depression is characterized by a long course of disease and repeated attack, which coincides with the concept of "deficiency" in TCM. Tonifying deficiency is a method for treating depression based on TCM syndrome differentiation. Under the guidance of this principle, the formulated prescriptions will produce the desired effects. This article reviewed the related clinical and experimental studies on depression treatment via deficiency tonification in recent years, and summarized the corresponding prescriptions for benefiting Qi, replenishing blood, nourishing Yin, and tonifying yang, respectively, so as to enrich the TCM theory of depression and provide new ideas for its clinical treatment.

ABSTRACT

Objective: To evaluate the effect of Haima Bushen Pills (HBP) on nourishing yin and reinforcing kidney, and study the mechanism of the action with aim to provide the experimental evidence for the clinic. Methods: Rats model with kidney-yang deficiency was induced by hydrocortisone through intramuscular injection to observe the anti kidney-yang deficiency effect of HBP. The effect of HBP against kidney-yin deficiency was observed using the kidney-yin deficiency model induced by oral administration of thyroxine and reserpine. The effect of HBP on erectile function was observed by intramuscular injection of hydrocortisone in rats. Castrated male rats and normal young male rats were administered orally to observe the androgenic effect of HBP. Results: After 10 d of administration, HBP at dose of 1.1 g/kg and 2.2 g/kg significantly increased the number of autonomic activities of kidney-yang deficiency mice, extended the duration of low-temperature swimming, and increased the organ coefficients of thymus and spleen. It also significantly reduced the number of platform jumping errors and prolonged the incubation period of errors, significantly increased the pain threshold and increased the organ coefficients of thymus and spleen in mice with kidney-yin deficiency. After 2 weeks of administration, HBP at doses of 0.56 g/kg and 1.12 g/kg significantly increased the bR/rR ratio and intravitreal pressure in rats, significantly improved the erectile function, significantly increased the serum testosterone (T) concentration and the viscera coefficients of prostate, foreskin gland and levator ANI of castrated rats. The serum concentration of testosterone (T), follicle-stimulating hormone (FSH) and viscera index of seminal vesicle were significantly increased in normal young rats after administration of HBP. Conclusion: HBP has obvious effect on nourishing yin and reinforcing kidney.

ABSTRACT

Objective To observe the effect of Ziyin-Jianghuo formula in treatment of Yin deficiency generating intrinsic heat type of the levels of monoamine neurotransmitters and estrogens in rats, and investigate its intervention in the neuroendocrine system. Methods There were 7 groups, which were sham operation group, model control group, estrogen tablet group, Gengnian capsule group, Ziyin-Jianghuo formula low, medium and high dose groups. Castration was performed by castration (extraction of ovaries) plus hot traditional Chinese medicine. The rats in the treatment group were given the above drugs 24 hours after the last administration of the hot Chinese medicine, once a day for 30 consecutive days. The sham operation group and the model control group were given an equal volume of purified water by simultaneous intragastric administration. The serum estradiol (E2), luteinizing hormone (LH), follicle stimulating hormone (FSH), prolactin (PRL) levels,uterine coefficient and monoamine neurotransmitters dopamine (DA), norepinephrine (NE), serotonin (5-HT), 5-hydroxyindoleacetic acid (5-HIAA) content in the hypothalamus were detected after 12 hours of the last treatment. Results Compared with the model group, the levels of serum FSH (4.39 ± 0.22 IU/L, 2.89 ± 0.91 IU/L, 2.84 ± 0.98 IU/L vs. 5.51 ± 0.24 IU/L), LH (14.48 ± 0.24 IU/L, 11.46 ± 0.33 IU/L, 5.28 ± 1.31 IU/L vs. 15.02 ± 0.37 IU/L) in the low, middle and high doses of Ziyin-Jianghuo formula significantly decreased (P<0.05), the levels of serum E2 (39.84 ± 6.08 pmol/L, 48.65 ± 6.77 pmol/L, 64.96 ± 7.97 pmol/L vs. 33.16 ± 4.62 pmol/L) significantly increased (P<0.05). The content of DA, 5-HT, 5-HIAA in the hypothalamus and the 5-HT/NE (0.48 ± 0.02, 0.43 ± 0.03, 0.27 ± 0.02 vs. 0.67 ± 0.02), 5-HIAA/5-HT (1.74 ± 0.09, 1.71 ± 0.10, 1.80 ± 0.17 vs. 2.00 ± 0.10) in the low, middle and high doses of Ziyin-Jianghuo formula significantly decreased ( P<0.05), the content of NE (663.34 ± 9.81 ng/kg, 695.94 ± 10.54 ng/kg, 790.76 ± 16.35 ng/kg vs. 602.95 ± 13.24 ng/kg) in the hypothalamus in the low, middle and high doses of Ziyin-Jianghuo formula significantly increased (P<0.05). The levels of serum PRL (10.16 ± 1.26 μg/L, 7.22 ± 1.26 μg/L vs. 14.80 ± 1.64 μg/L) in the middle and high doses of Ziyin-Jianghuo formula significantly decreased (P<0.05). Conclusions The Ziyin-Jianghuo formula has a significant positive regulation effect on the neuroendocrine system of menopausal Yin deficiency generating intrinsic heat type of rats, and this process is dose-dependent and can improve a series of symptoms caused by autonomic dysfunction.

ABSTRACT

The Chinese herbal Radix Scrophulariae is the main medicine for nourishing yin and reducing fire. It can be used to treat hyperthyroidism due to yin deficiency and fire hyperactivity, but its mechanism is not clear. The present study was aimed to explore the mechanism of Radix Scrophulariae treatment of hyperthyroidism due to yin deficiency and fire hyperactivity. The urine metabolomic approach was conducted using the method of UPLC-TOF-MS. The results showed that Radix Scrophulariae has good therapeutic effects on hyperthyroidism rat model of yin deficiency. After treatment with Radix Scrophulariae, through metabolic profiling and protocol analysis, 6 potential metabolic markers may be closely related with the treatment mechanism of Radix Scrophulariae on this disease, including proline betaine, estrone, thymidine, 5-hydroxyindoleacetic acid, cyclic AMP and L-dopa. The strongest metabolic pathways were associated with tryptophan metabolism, pyrimidine metabolism, tyrosine metabolism, purine metabolism and steroid hormone biosynthesis. The urine metabolomic approach can be applied to clarify the therapeutic mechanism of Radix Scrophulariae on hyperthyroidism rat of yin deficiency, and provide the theoretical basis for the clinical practice of Radix Scrophulariae on nourishing yin to reduce pathogenic fire.

ABSTRACT

Objective To observe the clinical effect of the method of Shugan-Yiqi-Yangyin treatment for the steroid-resistant nephrotic syndrome (SRNS). Methods A total of 80 patients with SRNS were divided into 2 groups by random number table method, 40 in each group. The control group received glucocorticoids combined with Tripterygium Glycosides;and the treatment group received Shugan-Yiqi-Yangyin on the basis of the control group, 3 month as a course. The 24 h urine protein quantitative (propagated), plasma albumin, blood lipid (total cholesterol, triglycerides), hemorrheology, blood urea nitrogen (BUN), creatinine (Cr), urinary inhibition C (Cys C) were detected before and after the treatment of two groups, and the clinical effect was compared. Results The total effective rate of the treatment group was 92.50% (37/40) and 82.50% (33/40) in the control group, and the difference was statistically significant (Z=-1.966, P<0.05). After the treatment, the 24 hPRO (1.03 ± 0.64 mg vs.2.81 ± 1.43 mg,t=3.025),Cys C(0.35 ± 0.41 mg/L vs.0.76 ± 0.51 mg/L, t=3.058) of the treatment group was significantly lower than those of the control group(P<0.05).The Alb(34.88 ± 2.17 mg vs. 31.69 ± 2.05 mg, t=2.986) of the treatment group was significantly higher than this of the control group (P<0.05), After treatment,the whole blood high shear viscosity(7.84 ± 1.42 mPa?s vs.8.94 ± 1.38 mPa?s,t=3.160),the whole blood low shear viscosity(4.55 ± 0.37 mPa?s vs.5.02 ± 0.44 mPa?s,t=3.825),plasma viscosity(1.33 ± 0.10 mPa?s vs.1.95 ± 0.26 mPa?s,t=2.981),hematocrit(0.28 ± 0.03 vs.0.34 ± 0.03,t=2.993),fibrinogen(3.96 ± 0.57 g/L vs.4.52 ± 0.47 g/L,t=4.863)of the treatment group were significantly lower than those of the control group(P<0.05).The TC(5.04 ± 1.72 mmol/L vs.6.99 ± 1.06 mmol/L,t=3.67),TG(1.4 ± 0.64 mmol/L vs.2.02 ± 0.31 mmol/L, t=3.040) of the treatment group were significantly lower than those of the control group (P<0.05). Conclusions The Shugan-Yiqi-Yangyin treatment for SRNS can obviously improve the symptoms,reduce the side effects of hormone of antagonism. The possible mechanisms are to restore kidney function, improve blood viscosity and lower blood lipid levels.

ABSTRACT

Objective To evaluate the clinical efficacy of phlegm and nourishing Yin combined with glucocorticoids in the treatment of refractory Mycoplasma pneumoniae pneumonia in children .Methods 134 children with Mycoplasma pneumonia were divided into control group and observation group according to the random number , 67 cases in each group .The control group were given hydrocortisone ,and the observation group received phlegm and nourishing Yin combined with glucocorticoid .After intervention ,the clinical data and the improvement of the clinical symptoms of the two groups were compared .Results There was no statistically significant difference in antipyretic time between the two groups(P>0.05).The time of cough relief,the absorption time of lung rales,the disappearance time of pleural effusion of the observation group were (3.61 ±1.13) d,(6.52 ±1.90) d and (7.02 ±2.10) d, respectively,which were significantly shorter than those of the control group [(4.89 ±2.18)d,t=2.920,P=0.021;(8.24 ±2.75)d,t=3.108,P=0.000;(9.26 ±2.38)d,t=5.027,P=0.000].There were no statistically signifi-cant differences in the total white blood cell count ,C-reactive protein and erythrocyte sedimentation rate between the two groups(all P>0.05).The percentage of lymphocytes in the observation group [(55.25 ±4.91)%] was signifi-cantly higher than that in the control group [(50.16 ±4.83)%,t=2.597,P=0.037].The effective treatment rate in the observation group was 97.01%,which was significantly higher than 83.58%in the control group (χ26.849,P=0.009).Conclusion Heat phlegm and nourishing Yin combined with glucocorticoid can not only effectively improve the patients'clinical symptoms , improve the proportion of lymphocyte , but also can improve the overall efficacy of children,it is worthy of clinical promotion .

ABSTRACT

Objective To investigate the effect of Nourishing Yin and Nonifying Yang sequential therapy (NYNYST )com-bined with western medicine on expression levels of Smad2 ,Smad3 ,Smad7 mRNA in rats with diminished ovarian reserve (DOR).Methods Totally 40 female rats were randomly divided into 5 groups ,the normal control group ,the model group ,the western medicine group ,the NYNYST group and the combination group(western medicine combined with NYNYST ) ,8 in each group. The DOR model was established through orally administering tripterygium pill for continuous 2 weeks. The normal con-trol group and the model group were administered with saline for 10 days. The western medicine group was treated with hor-mone replacement therapy(HRT ) and ovarian stimulation. The NYNYST group was administered with Nourishing Yin herbs in proestrus and Nonifying Yang herbs in late estrus and the combination group was administered with combination of Chinese herb and western drugs for 10 days ,with the same dose in the former two groups.Serum levels of FSH ,LH ,E2 ,anti-Müllerian hormone(AMH) ,Transforming growth factor beta 1(TGF-β1)and Inhibin B(INHB)were measured by ELISA.Changes of Smad2 ,Smad3 ,Smad7 mRNA in ovaries were detected by real-time PCR.Results Compared with the model group ,the serum levels of FSH ,LH were decreased significantly in western medicine group ,NYNYST group and combination group(P<0.01) , the serum levels of E2 ,AMH ,TGF-β1 and INHB increased in the rats of the treatment group(P<0.05 ,P<0.01) ,and efficacy of the combination group was significantly superior to that of the western medicine group (P<0.01 ,P<0.05);compared with the model group ,Smad2 mRNA increased significantly in NYNYST group and combination group ,Smad3 mRNA increased sig- nificantly in combination group(P<0.01) ,the efficacy of combination group was superior to that of the western medicine group (P<0.05);compared with the model group ,Smad7 mRNA of treatment groups was decreased significantly (P<0.01).Conclu-sion NYTYST combined with western medicine can improve the function of ovaries by up-regulating the expression of Smad2 , Smad3 mRNA and down-regulating the expression of Smad7 mRNA in DOR rats.

[Objective] To explore and analyze Professor ZHANG Yonghong's clinical experience in treating cerebral tumor.[Methods]Through the clinical study with the teacher to collect and collate the relevant information and medical records, from the etiology and pathogenesis of cerebral tumor, governance and other aspects of the specific analysis and discussion on Professor ZHANG Yonghong's treatment of this disease, academic point of view and clinical experience,it summed up the law of condemnation, and to the case of evidence,through regular follow-up, observing the treatment effect. [Results]Professor ZHANG Yonghong views that the pathogenesis of cerebral tumor can be summarized as Qi deficiency, phlegm and blood stasis, internal movement of the liver, and confusion of the body. Its pathologic factors are related to wind, fire, phlegm, blood stasis and poison. In the treatment of cerebral tumors, it is advocated to improve Qi, nourish Yin, promote positive Qi, and break loose to remove cancer toxins. On the basis of the selection of invigorating the spleen and protecting the stomach, the rational use of invigorating the Qi and nourishing the Yin and combining with the drugs of eliminating evils and removing toxin, in order to achieve the goal of strengthening the anti -cancer. The two cases both achieved good results by strengthening the body resistance, eliminating evils and removing toxin.[Conclusion]Professor ZHANG Yonghong stressed that strengthening of the body resistance is a primary treatment when the treatment for cerebral tumor. We should pay attention to replenishing Qi and nourishing Yin, eliminating evils and removing toxin are adjuvant therapy. The spleen and stomach should be protected throughout the treatment, and patient's mood are also important. It is of great significance to instruct TCM to treat cerebral tumor.

ABSTRACT

[Objective] This paper introduces the experience of Professor TAN Yong's treatment of the luteinized unruptured follicle syndrome(LUFS) with a view to provide references for the related study.[Methods]Through studying with Professor TAN, collecting and analyzing clinical case data, researching editorial book and articles. From the etiology, pathogenesis and drug selection of the LUFS, the clinical experience of the teacher was discussed, Professor TAN's academic characteristics on the treatment of LUFS were summarized, and a medical case was provided to support the idea.[Results]Professor TAN believes that the pathogenesis of this disease is mostly due to deficiency of the kidney Yin, the weak Yang qi, Yin and Yang conversion is unfavorable during intermenstrual period, combined with pathological factors such as liver depression, blood stasis, and phlegm dampness. Unsmooth running of Qi and blood, so that the follicular can not be discharged smoothly, the disease occurs. In the treatment of combination of Traditional Chinese Medicine(TCM) and western Medicine, in the intermenstrual period attach importance to the function of the heart and brain, to tonifying kidney and blood circulation as a method, the use of "Tonifying Kidney Stimulating Ovulation" medicine treatment, combined with western medicine, in the clinically achieved good results. The medical case was based on the "Tonifying kidney Stimulating Ovulation" medicine as the basis for the successful pregnancy after the treatment, which fully proved the above mentioned dialectical thinking and treatment experience. [Conclusion]It is worthwhile to learn and promote the treatment of LUFS with tonifying kidney and blood circulation therapy.

This paper will discuss the basic research and clinical research on the establishment of theanimal model of yin deficiency, the detection of physiological and biochemical indexes, yin deficiency constitution andTCM in the prevention and treatment of yin deficiency to provide data to clarify the material basis of yin deficiency andlay the foundation for the standardization and unification of the syndrome differentiation and treatment formulation of clinical TCM.

ABSTRACT

OBJECTIVE:To discuss the effect and mechanism of supplementing qi and nourishing yin(SQNY) decoction on intestinal microbiome of mice with Lewis lung carcinoma after treated with cisplatin. METHODS: The model of C57BL/6 mice with lung carcinoma xenograft was established by subcutaneous seeding of Lewis cells and randomly divided into 4 groups (n=16), including saline group (NC group), cisplatin group (DDP group), CAP group (CAP group) and SQNY decoction combined with cisplatin group (DDP+SQNY group). The tumor sizes, weights and intestinal microbiota were detected. The serum levels of IL-6, IL-10, and TNF-α were detected by ELISA. The dendritic cell (DC) and macrophage subtype in spleen of mice models were detected by flow cytometry. RESULTS: ①The mean tumor sizes of mice in DDP group, CAP group, DDP+SQNY groups were all smaller than NC group (P<0.05). And the mean tumor size and weight in DDP+SQNY group were less than the other three groups (P<0.05). The mean animal weights in NC or DDP+SQNY group were higher than those in DDP group and CAP group (P<0.05). ②The mean levels of serum IL-6, IL-10 and TNF-α in DDP+SQNY group were less than those in the other three groups (P<0.05). ③The mean microbiota richness in DDP+SQNY group were less than those in the other three groups (P<0.05). The proportion of fecal microbiota was significantly changed in DDP+SQNY group, including upregulation of Firmicutes phylum and Bifidobacterium Genus, and downregulation of Bacteroidetes phylum(P<0.05). The amount of DC and M1/M2 macrophage subtype in spleen of DDP+SQNY group mice models were more than those in the other three groups (P<0.05). CONCLUSION: It′s suggested that SQNY decoction effectively enhances the anticancer effect of cisplatin by increasing the immune function and changing the intestinal microbiota.

Objective To assess the clinical therapeutic effects and safety of Jiangtang Shuxin decoction (JTSXD) on diabetic patients complicated with chronic heart failure (CHF),and to search for its possible function mechanisms.Methods A prospective randomized controlled study was conducted,80 diabetic patients complicated with CHF [New York Heart Association (NYHA) functional class Ⅱ-Ⅲ] admitted into the Department of Traditional Chinese Medicine (TCM) or of Cardiology in Affiliated Hospital of Guangxi Youjiang National Medical College from October 2015 to September 2016 were enrolled,they were assigned to an observation group and a control group by randomized method with a computer,and finally 77 patients (39 cases in observation group and 38 cases in control group) completed this trial.The patients in control group received standardized routine western medical treatment,while the observation group was additionally administered JTSXD (including ingredients:astragalus 15 g,ginseng 10 g,radix ophiopogonis 15 g,radix rehmanniae 15 g,comus 10 g,rhizome coptidis 8 g,peach kernel 10 g,salvia mitiorrhiza 10 g,magnoliaceae 10 g,yam 15 g) on the basis of conventional therapy.The therapeutic course for all the patients in both groups was 2 months.Before and after treatment,the 6-minute walking distance (6MWD) was assessed;the TCM syndrome accumulated scores of the two groups were calculated;the left ventricular end-diastolic volume (LVEDV),the left ventricle ejection fraction (LVEF),the stroke volume (SV),the cardiac output (CO),and the maximum blood flow velocity of early diastolic/atrium late diastolic (E/A) were detected by echocardiography.The serum levels of glycosylated hemoglobin (HbA1c),angiotensin Ⅱ (Ang Ⅱ) and plasma B type brain natriuretic peptide (BNP) were tested with enzyme linked immunosorbent assay (ELISA);the level changes of total cholesterol (TC),triglyeride (TG),high density lipoprotein cholesteral (HDL-C) and low density lipoprotein cholesteral (LDL-C) were observed.Results Compared with the control group,after treatment in the observed group,the TCM syndrome score of palpitation,fatigue and thetotal accumulated score were all obviously decreased (palpitation score:0.9 ± 0.4 vs.1.2 ± 0.8,fatigue score:1.1 ± 0.7 vs.1.7 ± 0.8,total accumulated score:4.8 ± 1.2 vs.8.1 ± 1.8,all P < 0.05);the LVEDV,the serum levels of HbA1c,Ang Ⅱ and BNP were also obviously decreased in the observed group [LVEDV (mL):136.28 ± 17.52 vs.158.82 ± 19.03,HbA1c (%):6.11±0.36 vs.6.89 ±0.32,Ang Ⅱ (ng/L):66.48 ± 17.64 vs.84.55 ± 20.39,BNP (μg/L):138.45 ± 87.55 vs.219.14±88.83,all P < 0.05];The 6MWD,LVEF,SV,CO and E/A were all increased plainly in the observed group [6MWD (m):470.47 ± 79.66 vs.428.46 ± 88.56,LVEF:0.51 ±0.05 vs.0.46 ± 0.04,SV (mL):55.36 ± 2.88 vs.50.32±2.76,CO (L/min):5.74±0.91 vs.4.92±0.74,E/A:1.18±0.27 vs.0.83±0.28,all P < 0.05].The degrees of decreased levels in TC,TG,LDL-C and the degrees of increased levels of HDL-C in observed group were superior to those of the control group,but there were no statistical significant differences (all P > 0.05).Conclusion JTSXD shows good therapeutic effect and safety for treatment of diabetic patients accompanied by CHF (NYHA functional class Ⅱ-Ⅲ),and its mechanisms may be related to its regulation of glucose (reduction of HbA1c level),correction of lipid metabolism disorders,improvement of myocardial energy supply,inhibition of the activation of renin-angiotensin-aldosterone system (RAAS) and the secretion of BNP.

At present, the prevalence rate of diabetes presents a rising tendency. The cardiovascular disease is a major complication of diabetes mellitus and acute coronary syndrome (ACS) is a severe form of coronary heart disease. Compared with non-diabetic patients, the disease situation in diabetic ACS patients is more serious with more contradictory problems and difficulty in treatment. Although percutaneous coronary intervention (PCI) plays a certain role in re-canalization of coronary artery, after PCI the problems the patients with diabetic ACS will face are still serious. traditional Chinese medicine (TCM) recognizes that the etiology and pathogenesis of diabetic ACS are deficiency in Qi and Yin, and accumulation of blood stasis and toxin inside the body. Through clinical observation and retrospective analysis, it is found that using "supplementing Qi and nourishing Yin, activating blood circulation and detoxifying"can interfere with the patients' diabetic ACS and simplify the therapeutic regimen, which is an integrated regulatory treatment with multiple links, multiple pathways and multiple targets, and is a sufficient realization of unique superiority of traditional Chinese medicine by using holistic concept, syndrome differentiation and multiple-link interference for comprehensive prevention and treatment of diabetic ACS.

ABSTRACT

Objective To compare the clinical efficacy of acupuncture and medicine for the treatment of chronic pharyngitis. Methods Sixty chronic pharyngitis patients were randomized into treatment group and control group, 30 cases in each group. The treatment group was treated with acupuncture mainly on acupoints of Zhaohai(KI6), Lieque(L7), Tiantu(CV22) , Lianquan(CV23), Tianrong(SI17) , Hegu(LI), Yuji (LU10) for nourishing yin to reduce fire and clearing throat. The supplementary acupoints were selected according to the symptoms and physical signs. Acupuncture was performed once a day and 6 times a week. The control group received western medical therapy including pharyngeal application with 1% iodine glycerin preparation and oral use of Cydiodine tablets, 3 times per day. Four weeks constituted one treatment course and the treatment for the two groups covered 2 courses. After treatment, the therapeutic effect was evaluated. The scores of the signs and s ymptoms as well as scores of chronic pharyngitis syndrome discomfort rating questionnaire(CPSDQ) evaluated by visual anal og scale(VAS) were observed. Results (1) The overall effective rate of the treatment group was 93.3%, higher than that of the control group(73.3%), and the inter-group difference was statistically significant (P<0.05). (2) After treatment, scores of the signs and symptoms and VAS for CPSDQ scores in the two groups were reduced as compared with those before treatment(all P<0.05 or P<0.01 ) .The improvement of the scores in the treatment group was superior to that in the control group(all P<0.05 or P<0.01).(3) During the treatment , needling-induced mild pain and bleeding occurred in the treatment group. No acupuncture syncope, stuck needles or allergic and toxic-side effect was shown in the two groups. Conclusion Acupuncture therapy can obviously relieve the symptoms, signs and discomfort in the patients with chronic pharyngitis, and the curative effect is superior to the drugs.

ABSTRACT

Objective To observe the clinical curative effect and the serum CPR changes by the treatment principle of Yangyin-Huayu for the patients with recurrent hemoptysis of Bronchiectasis. Methods A total of 64 patients were enrolled and divided into 2 groups according to the random number table method, 32 patients in each group. The control group was treated with the injection of blood clotting enzyme, pituitrin, levofloxacin lactate. And the treatment group was treated with the Yangyin-Huayu on the basis of the control group. Two groups were treated for 10 days. The CPR was detected by the double optical path immune turbidity analyzer. And the clinical effects, symptoms and signs scores were compared between before and after treatment and between two groups. Results Compared with the total effective rate of control group 84.38%(27/32), the treatment group was 96.88% (31/32) and significantly high (χ2=6.372, P<0.01). After treatment, the levels of serum CPR (2.45 ± 1.42 mg/L vs. 6.19 ± 2.34 mg/L, t=4.671) in the treatment groupwas significantly lower than that in the control group (P<0.01). The scores of cough (0.62 ± 0.91 vs. 1.34 ± 0.58, t=5.364), sputum (0.14 ± 0.28 vs. 0.68 ± 0.72, t=4.035), haemoptysis (0.10 ± 0.65 vs. 0.62 ± 0.26, t=5.012), fever (0.04 ± 0.31 vs. 0.76 ± 0.07, t=3.026), breathing difficulties (0.12 ± 0.41 vs. 0.69 ± 0.05, t=4.010) in the treatment group were lower than those in the control group (P<0.01). Conclusions The treatment principle of Yangyin-Huayu combined with conventional western medicine therapy can improve the patients with recurrent hemoptysisin the fields of symptoms, signs, the serum level of CPR, and total effect rates.

ABSTRACT

Objective To study the effect of Nourishing-Yin Removing-Fire and Dissipating-Stagnation Decoction on intrathyroid vascularity( ITV) and serum vascular endothelial growth factor(VEGF) levels in patients with new diagnoses of Graves disease(GD).Methods 100 patients with new diagnoses of GD were divided into two groups:Group A were treated with Nourishing-Yin Removing-Fire and Dissipating-Stagnation Decoction combined with methimazole(MT) and Group B were treated with MT alone.Thyroid volumes, superior thyroid artery peak systolic velocity(STA-PSV), thyroid vascularity index(TVI) and serum VEGF levels in patients were also observed in Group A and B before and after treatment.The correlation between thyroid volumes and serum VEGF levels in patients was analyzed.Results The parameters of GD were significantly higher than those in control group (P<0.01) .There were positive correlation between thyroid volumes and thyroid vascularity index(TVI) or serum VEGF levels in patients(r=0.57, P<0.05;r=0.59,P<0.05).The parameters in Group A and Group B decreased after treatment(P<0.01).STA-PSV, TVI and serum VEGF levels decreased significantly in Group A than those in in Group B after treatment(all P<0.05).Conclusion Nourishing-Yin Removing-Fire and Dissipating-Stagnation Decoction can improve goiters more effectively than traditional western medicine alone by decreasing serum VEGF levels of patients with GD.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effect of Chinese herbal medicines for nourishing yin, supplementing qi, and activating blood on the reproductive endocrine-immune network and its mechanisms in patients with primary Sjogren's syndrome (pSS).</p><p><b>METHODS</b>Seventy pSS patients were randomly assigned to two groups using a randomized digital table: the integrative therapy group (36 cases) and the control group (34 cases). Thirty healthy subjects were taken as a normal group. The control group was treated with hydroxychloroquine sulfate tablets alone, and the integrative therapy group was treated by Chinese herbal medicines for nourishing yin, supplementing qi, and activating blood combined with hydroxychloroquine sulfate tablets. The treatment course was 6 months for both groups. Before and after treatment, serum estradiol (E2), testosterone (T), luteinizing hormone (LH), prolactin (PRL) by radioimmunoassay and immunoglobulin (IgG) by immunodiffusion, erythrocyte sedimentation rate (ESR) by Westergren, interferon-γ (IFN-γ) and interleukin-4 (IL-4) by enzyme linked immunosorbent assay were determined.</p><p><b>RESULTS</b>E2 and T levels in all patients were lower than those of normal subjects before treatment (P<0.05) and were increased significantly after 6-month treatment (P<0.05). ESR, FSH, LH, IgG, IFN - γ, IL - 4 and ratios of E2/T, and IFN -γ/IL in the patients were higher than those of normal subjects before the treatments (P<0.05), and were reduced significantly after the treatments (P<0.05). The T and IFN - γ levels and E2/T ratio in the patients treated with integrative therapy were reduced significantly compared with the control group (P<0.05). However, the PRL levels before and after treatment were not significantly changed in the two groups (P>0.05). The ratios of E2/T and IFN -γ/IL-4, and levels of IgG and ESR were positively correlated before and after treatment (P<0.05).</p><p><b>CONCLUSIONS</b>The ratios of E2/T and IFN -γ/IL-4 might be used as indicators of pSS activity. Chinese herbal medicines for nourishing yin, supplementing qi, and activating blood combined with Western medicine could improve the therapeutic effect by regulating the reproductive endocrine-immune network in pSS patients.</p>
